Fam Name (M)
Type the family name of the student.
Given (M)
Type the given name of the student.
Stud Name
Type a preferred given name if applicable.
D.O.B. (M)
Type the date of birth of the student in dd/mm/yy format.
Date of Reg (M)
Type the date of the student's initial registration at the school.
Gender (M)
Type the gender of the student.
Pref Yr Comm
Type the calendar year the student would prefer to commence.
Pref Yr Level
Type the year level the student would prefer to commence at.
Sibling
Click this tick box if the student has a sibling at the school.
Eldest in Family
Click this tick box if the student is the eldest in the family.
School Card
Click this tick box if the student has, or is eligible for, a school card.
Acceptance
Click this tick box if the student has been accepted into the school.
P/G 1 Occupation
Type the occupation of the first parent/guardian.
P/G 2 Occupation
Type the occupation of the second parent/guardian (if one exists).
Address
Type the student's full mailing address. At least one address line must be completed.

Type or double click to select the student's mailing suburb/town and postcode.
Bill Address
Type the student's full billing address.
