Field names and descriptions
Sibling
Click this tick box if the transferring student has a sibling at the out-of-district school.
Name
If the Sibling tick box is selected, the sibling's name must be entered.
Medical
Click this tick box if the transferring student has a medical condition.
Curriculum
Click this tick box if the transferring student has special curriculum needs. Up to four curriculum needs may be flagged.
Other
Click this tick box if the transferring student has other special needs. Up to four other needs may be flagged.
