Displays the student identification number.
Referred By >>
Double click to select the referring professional support required.
Impairment 1 >>
Double click to select the student's impairment.
Impairment 2 >>
Double click to select the student's second impairment (if applicable).
Impairment 3 >>
Double click to select the student's third impairment (if applicable).
State Provider >>
Double click to select the state support provider (if applicable).
Regional Provider >>
Double click to select the regional support provider (if applicable).
Other Provider >>
Double click to select another support provider (if applicable).
Curr Sup Lev >>
Double click to select the current level of support provided.
Level Next Year >>
Double click to select the level of support which is expected to be provided for the following year.
Comm Mode 1 >>
Defaults to the communication mode of verbal. If another communication mode is required, double click to select another.
Comm Mode 2 >>
Double click to select an alternative communication mode that can be used with this student.
Exp Transition >>
Double click to select the expected transition for this student (if applicable).
Exit Code >>
Double click to select the reason for this student exiting the school system.
